Frequently asked questions
Is the Toolkit free?
Yes. Read it right here on any device and share it with your team.
Do I need an account to use the tools?
No. The tools open right inside the book, with no sign-in. What you type saves only on your own device.
Where do we go for the whole transition timeline?
The Transition Roadmap is Barb's complete family guide to transition planning from age 14 to adulthood.
Key terms, explained simply
- Transition planning: planning for life after high school: education and training, work, independent living and community life. It belongs in the IEP by age 16, and many families start at 14.
- Adult services: supports that begin after school, such as Vocational Rehabilitation, developmental disabilities services, SSI and Medicaid. Many have waitlists, so applying early matters.
- Self-determination: making choices and having a say in your own life. Young adults with stronger self-determination skills have better outcomes after school.
- Supported decision-making: an alternative to guardianship where a young adult makes their own decisions with help from people they trust.
